Registration

To Register at Lawnside school several forms must be completed and submitted to complete enrollment.

 Pick up a packet in the main office

 complete the forms, and call for an appointment

1. Proof of residiency

2. Documentation of relationship to the student

3. Documentation of grade placement from prior school district

4. Health Documents:

Orginal copy of Immunizations for out of state, pre-k or K students

Current physical within 12 months

Copy of orginal health card from prior school district.
