MANDATORY VACCINES FOR GRADE 6

     MANDATORY IMMUNIZATIONS REQUIREMENTS

    FOR STUDENTS  ENTERING GRADE 6

       

 NEW JERSEY CODE N.J.A.C. 8:57-4 IMMUNIZATION OF PUPILS IN SCHOOLS

requires pupils entering grade 6, or comparative age level class, and born after 1-1-1997

(age 11) to have documented evidence of one (1) dose of meningococcal vaccine and

one (1) dose of tetanus, diphtheria, and acellular pertussis (Tdap).

        Students are also required to receive a physical exam during pre-adolescense,

grade 4-6. Get a physical when you get your immunizations and meet both requirements.
