Mandated Immunizations

        Students entering a New Jersey Public School from a New Jersey school the parent    must provide a copy of the A-45 from the sending school.

       Students entering Lawnside Public School from an out of the state school the parent must provide must a copy of current immunizations and a physical exam current within 1 year.

• DTap: 4 doses (one given after the 4th birthday)      
• Polio: 3 doses (one given after the 4th birthday)
• MMR: 2 doses (any child born after 1990)
• Varicella: 1 dose (must be after the child's 1st birthday )
• Hepatitis B: 3 doses
• Pnemococccial: 1 dose ( age 12 mom to  4 yrs 9 mo.) 
• HIB: 1 dose (  age 12 mo to 4yr 9 mo)
• Menningocococcal: 1 dose ( if born after 1997) * mandated for grade 6
• Tdap: 1 dose (if born after 1997) *mandated for grade 6 
• Influenza: 1 dose annually

No student can be admitted without documentation of required immunizations
